How do you use and disclose my personal health information?
We use or disclose personal health information for the purposes of carrying out treatment, obtaining payment, evaluating the quality of services provided, and any administrative operations related to treatment or payment.
Where should I park?
2 hour metered parking is available outside of the clinic. Public parking garages are located on the corner of 4th and Broadway. Public parking garages are free for the first 90 minutes.
What to expect:
- Each session will be one-on-one with a Doctor of Physical Therapy.
- Each session will be 60 minutes.
- You will have access to your Physical Therapist after hours via our patient portal or email system. We are here for you, we want to know how you're doing with your recovery
- Rehabilitation/performance program development specifically tailored to your needs and goals.
Do I need a prescription or referral to see a physical therapist?
No. In the state of CA, a patient does not need a prescription or a referral from a physician before seeing a physical therapist. You may go directly to the therapist and start treatment right away. However, you should always check with your health insurance provider to determine if they allow for "direct access" as some companies require a physician's prescription before starting treatment.
Why don’t you accept my insurance?
Traditional insurance models limit both the patient and the provider. Removing the middleman allows us to treat you in the way that will best suit your needs without the headaches and restrictions imposed by insurance companies.
How much will my sessions cost?
Cost transparency is important to us. You will know your financial responsibility prior to services being rendered.
Do you take health insurance?
We are In-Network with Medicare and Out-of-Network providers for all other insurances. This allows us to treat you the way we know how and not be restrained by insurance based models.
Can I use my FSA/HSA?
Yes, you can use your FSA/HSA account to pay for our services
Can I receive reimbursement from my insurance?
Yes, we will create a superbill that can be accessed in your patient portal and can be submitted by you to your insurance for reimbursement. Your sessions will count toward your deductible, however, always check with your insurance provider as some payers reimburse more or less for Out-of-Network services.
What is the difference between physical therapy and Performance/Fitness services?
Traditional physical therapy addresses patients with physical impairments related to an injury, disease or condition. Performance/fitness is a service provided by the same therapists with the aim of keeping you healthy, injury-free while optimizing your performance.
What should I wear?
Anything you're comfortable in that can allow for easy exposure of the targeted areas. Most people wear athletic/loose fitting clothing
